Covered Medications
A prescription medication means any medication that may
be legally dispensed only when you have a written prescription
from a physician or other licensed medical provider.
The physician may also call in a prescription directly to
your local pharmacy or the Caremark® mail service pharmacy.
Insulin and other supplies for diabetes, such as syringes,
needles and testing materials, are also covered.
When you get a prescription, the pharmacist will fill it
with a generic medication unless no generic drug exists
for that brand or the prescribing physician has specified
“Dispense As Written” (DAW). If there is no generic
equivalent for a brand name drug, the brand name will be
provided as ordered and covered under the Plan. If there is
a generic equivalent, the cost of the prescription will be
subject to the $500 deductible.
