Notice of a Qualifying Event
Under the law, you or your dependent are responsible for notifying MILA of the occurrence of certain qualifying events under which you or your dependent will lose coverage under the Plan. Those qualifying events are your divorce or your child ceasing to be an eligible dependent under the Plan.

*If you are called to active duty in the U.S. military, your dependents will continue to be covered under the Plan as described here.
You must notify MILA within 60 days of the date of the event or the date coverage would end under the Plan because of the qualifying event, whichever is later.
Your employer is responsible for notifying MILA of the occurrence of one of these qualifying events:
- Your death;
- The termination of your employment;
- The reduction in your hours of employment; or
- Your entitlement to Medicare.
The time period in which your employer must notify MILA of one of these qualifying events commences on the date of the loss of coverage. Once MILA is notified of any of these events, MILA will send you a COBRA notice as soon as possible, but in no event later than 44 days.
