WHAT IS COVERED

Outpatient Surgery

Outpatient surgery refers to non-emergency surgery that is performed in a doctor’s office, clinic or the outpatient facility of a hospital. Generally, outpatient surgery requires advance approval. Although an In-Network provider will get the approval for you, it’s your responsibility to make sure the surgery is approved. If you use an Out-of-Network provider, you must get the necessary approval yourself. For a list of outpatient surgeries that require approval, click here.
