Notice of a Qualifying Event
Under the law, you or your dependent are responsible for notifying MILA of the occurrence of certain qualifying events under which you or your dependent will lose coverage under the Plan. Events are the loss of coverage due to (1) your divorce and (2) your child ceasing to be an eligible dependent under the Plan. If you lose coverage due to your reduction in hours, termination of employment or death, MILA must be notified within 60 days of the later of: (1) the date of the event or (2) the date coverage would end under the Plan because of the Qualifying Event. Your employer is responsible for notifying MILA of the occurrence of one of these Qualifying Events:
- Your death;
- The termination of your employment; or
- The reduction in your hours of employment.
The time period in which your employer must notify MILA of one of these Qualifying Events commences on the date of the loss of coverage. Once MILA is notified of any of these events, MILA will send you a COBRA notice as soon as possible, but in no event later than 44 days after MILA receives notice of the Qualifying Event.
